Veritas Christian Academy

Before a student can participate in any sport, they must have this form filled out by their parents and a doctor. If a student participates in multiple sports in the same school year, they need to only fill the form out once. However, the form must be filled out and re-submitted every year. This is an essential form in case of an emergency.

In order to get busing to VCA or a reimbursement each family must fill out a B6T form every year and submit it to the office.

Students, parents, friends and businesses are encouraged to buy an ad in the yearbook to help pay for the yearbook. The ads will be read by the many families of the school and their extended families whom they share the yearbook with.

This packet is to be printed off, signed and returned to the office by new families who are notified that their son or daughter has been accepted to Veritas for the upcoming year. Please let the office know if you have any questions.
