Current or former students of Veritas Christian Academy or their parents can use this form to request a transcript. Please fill in the form below and we will send out the transcript as soon as possible. Once you have submitted the form you will be redirected to a confirmation page.
If you are requesting transcripts for more than one student, you will need to fill out this form for each student. If you are requesting more than one transcript for a student, you will fill out the form only once.
